• Coordinate care for clients with behavioral health conditions requiring intensive interventions
• Conduct comprehensive health risk assessments and develop tailored care plans
• Monitor and evaluate the effectiveness of care plans and interventions
• Collaborate with interdisciplinary teams to ensure quality care delivery
• Educate members, providers, and families on care coordination and health strategies

• 3-5 years of experience in Social Work, Nursing, or a Healthcare-related field, or relevant experience in lieu of degree
• Experience in utilization management, quality assurance, or community health
• Strong analytical skills to assess trends and make informed decisions
• Effective verbal and written communication skills
• Valid driver's license in the state of employment

• Associate or Bachelor's degree in a relevant field
• Certified Case Manager (CCM) or Licensed Clinical Social Worker (LCSW) certification
• Registered Nurse (RN) licensure in the state or compact state
• Experience with cost-effective care coordination and resource management
