Temporary 13‑week assignment, Monday‑Friday 8 am–5 pm, $65.00 per hour. PRN only position. Responsible for helping patients maintain, restore, and improve movement, activity, and function within a physician‑approved plan of care. Duties include interventions, re‑examinations, discharge planning, and coordination with the interdisciplinary team.
Responsibilities- Provide comprehensive patient examination and evaluation at the initial visit upon receipt of referral.
- Reassess the patient at a minimum of every 30 days, making necessary revisions to the patient’s plan of care in conjunction with the physician and documenting the patient’s continual care needs.
- Provide physical therapy patient/client management, including interventions in accordance with the physician‑approved plan of care.
- Document each visit according to the APTA Documentation Guidelines and accepted regulatory standards for inclusion in the patient’s clinical record.
- Coordinate care by participating in the interdisciplinary team and by observing, recording, and reporting the patient’s response to treatment and any change in the patient’s condition to the team.
- Identify the patient’s need for other disciplines and services, initiating appropriate referrals and providing resources for future needs.
- Act as a consultant to other care providers as needed.
- Complete OASIS assessments timely, accurately, and according to the OASIS user’s manual definitions.
- Supervise Physical Therapy Assistants (PTA) in accordance with applicable municipal, state, and federal rules and regulations.
- Provide consistent patient care oversight.
- Remain accessible by telecommunication, conduct regularly scheduled and documented conferences with the PTA, and make supervisory visits to the patient.
- Assess competence of PTA in performing assigned activities.
- Remain available upon request of the PTA for re‑examination of the patient in instances of a noted change in the patient’s medical status, response to interventions, or a discharge or discontinuation from physical therapy services.
- Include documentation of supervisory activities in the patient’s medical record.
- Participate in patient care conferences, in‑services, chart audit activities, quality improvement activities including OBQI and OBQM, and peer‑review activities.
- Maintain confidentiality of information relating to the physical therapist‑client relationship.
- Make recommendations for follow‑up care in coordination with the care center that admitted the patient and utilize other care center providers to assist with referrals to community resources.
- Supervise the Home Health Aide when the Sn is not active in the case. Communicate plan of care changes to the Aide and other appropriate staff as necessary.
- Keep abreast of physical therapy trends and knowledge for service provision, documentation, and care coordination.
- Comply with policies, procedures and regulatory mandates including but not limited to abiding by the terms of the Amedisys Compliance Program.
- Perform other related duties as assigned.
- Current, unencumbered license to practice physical therapy specific to the state of assignment.
- Graduate from an accredited physical therapy educational program as set by the Commission on Accreditation of Physical Therapy Education (CAPTE).
- Minimum of one (1+) year of Home Health physical therapy experience.
- Current CPR certification.
