Je hebt de verantwoordelijkheid voor zorgplannen en coördineert multidisciplinaire overleggen. Vereisten zijn onder andere een afgeronde mbo-opleiding en ervaring in de GGZ. Je ontvangt training en persoonlijke begeleiding, met opties voor een leaseauto en flexibele werkmethoden.
