Prodeba zoekt een ambulant begeleider voor regio Den Haag. Je biedt ambulante begeleiding aan jongvolwassenen met psychische- en/of ontwikkelingsstoornissen en bepaalt samen met de cliënt het zorgplan. Je werkt grotendeels zelfstandig en bezoekt cliënten op locatie.
Je bent verantwoordelijk voor de uitvoering en bijsturing van zorgplannen, hebt ruimte om te sparren in teamoverleggen en kunt desgevraagd bij een gedragsdeskundige terecht voor overleg. Zie jij dit als jouw uitdaging?
