WZH De Strijp is op zoek naar een Begeleider NAH om een nieuwe NAH-afdeling op te zetten en ouderen met Niet Aangeboren Hersenletsel te begeleiden. Je werkt in een multidisciplinair team en bouwt actief mee aan de afdeling.
Je ondersteunt in de ochtend bewoners bij ADL, stimuleert zelfstandigheid en begeleidt het dagprogramma, therapieën en sociale activiteiten. Gedurende de dag signaleer je veranderingen en rapporteer je bevindingen voor continuïteit van zorg.
