Joinuz Arbeidsbemiddeling & Advies zoekt een Wmo Consulent die inwoners helpt zo zelfstandig mogelijk mee te blijven doen. Je luistert, vraagt door en bekijkt wat er nodig is, van keukentafelgesprekken tot herindicaties, bij gemeenten waar jouw kennis direct van waarde is.
Je werkt volgens de Wmo, voert huisbezoeken uit, beoordeelt aanvragen en documenteert rapporten en beslissingen. Minimaal 2 jaar gemeentelijke Wmo-ervaring, een relevante HBO-opleiding zoals Social Work, Ergotherapie,
