Een zorgorganisatie in Den Helder zoekt een wijkverpleegkundige voor een tijdelijk dienstverband van 24 tot 32 uur per week. In deze rol ben je verantwoordelijk voor de zorgcoördinatie bij cliënten met dementie en werk je zelfstandig in de thuissituatie. Je hebt sterke communicatieve vaardigheden en kunt veranderingen in de situatie van cliënten signaleren. Een HBO-opleiding Verpleegkunde en geldige BIG-registratie zijn vereist.
