Job Overview

The Community Mental Health Team are seeking a skilled, motivated, organised and enthusiastic Mental Health Nurse to work within an established integrated, multi-disciplinary team, focusing upon individuals with severe and enduring mental health problems.

The successful post holder will undertake responsibility for the care co‑ordination and case management of a defined caseload of clients with complex and challenging needs as defined within Parts 2 and 3 of The Mental Health (Wales) Measure 2010.

The ability to speak Welsh is desirable for this post;
English and/or Welsh speakers are equally welcome to apply.

Core Responsibilities
  • Be responsible for care co‑ordination of a designated caseload of clients with severe and enduring mental health problems.
  • Ensure that Care and Treatment Plans are adhered to and meet standards in line with the Mental Health Measure (Wales) 2010.
  • Act as a point of contact for GPs and other interface services, offering advice, support and signposting to appropriate agencies.
  • Formulate and develop standard and complex, outcome‑focused care and treatment plans, and complete comprehensive risk assessments for actual and potential risk.
  • Support clinical duties such as assisting with the running of the depot/clozapine clinic and administering and supervising long‑acting injections, and complete physical health assessment and side‑effect monitoring for patients prescribed clozapine.
  • Assess individuals in crisis, completing comprehensive assessments of health and social care needs, agree an appropriate course of action, evaluate and manage clinical risk, formulate a plan of care and deliver evidence‑based crisis interventions.
  • Work collaboratively with service users, carers and partner agencies to deliver high‑quality care.
