Key Responsibilities- Be responsible for the care coordination of a designated caseload of clients with severe and enduring mental health problems.
- Ensure that Care and Treatment Plans are adhered to and that standards are met in line with the Mental Health Measure (Wales) 2010.
- Act as a point of contact for GP’s and other interface services to offer advice, support and signposting to appropriate agencies.
- Formulate and develop standard and complex outcome‑focused care and treatment plans.
- Ensure that comprehensive risk assessments are completed and documented for actual and potential risk.
- Support clinical duties such as assisting with the running of the depot/clozapine clinic and the administration and supervision of long‑acting injections. Successfully complete physical health assessments and side‑effect monitoring for patients prescribed clozapine.
- Assess individuals in crisis, completing comprehensive assessments of health and social care needs and agreeing the most appropriate course of action.
- Assess, evaluate and manage clinical risk.
- Formulate a plan of care and interventions based on identified need and deliver evidence‑based crisis interventions as appropriate.
- Work collaboratively with service users, carers and partner agencies in delivering high‑quality care.
- The ability to speak Welsh is desirable for this post;
English and/or Welsh speakers are equally welcome to apply. - Relevant nursing qualifications and experience in mental health settings.
- Experience with care coordination and case management of clients with complex and challenging needs.
- Familiarity with the Mental Health (Wales) Measure 2010.
- Strong communication, risk assessment and crisis intervention skills.
