Assessment​/Referral Coordinator -Acute Rehab - Part Time

Responsibilities

Texoma Medical Center, a 414‑bed acute care facility has been providing quality health care to the residents of North Texas and Southern Oklahoma since 1965. Our main campus is located in Denison, Texas, approximately one hour north of the Dallas/Fort Worth area and just south of the Texas/Oklahoma border. In addition, we have numerous facilities in locations throughout the Texoma region.

The basic purpose of nursing is to provide safe, competent nursing care for patients in accordance with facility policies, standards and philosophy. Additionally, nursing will provide professional leadership and support to the healthcare team. The concepts of Patient Centered Care will provide the foundation for all nursing care. Demonstrates Service Excellence standards at all times. The Standards of Practice set forth by the professional nursing associations will provide guidelines for the competent, safe and professional practice.

The RN Evaluator/Marketer has multiple roles including caregiver, decision maker, client advocate, marketer, and educator. The functions include providing marketing, community support, and customer service activities in representation of TMC RRH. Participates in referral source identification, assessing the needs of the customer and making the requested number of marketing contacts each month in order to develop and maintain long term referral relationships to generate a continued flow of admissions to meet RRH census objectives.

Functions also include collecting information about the client needs and development of an appropriate plan relevant to the rehabilitation setting. He or she plans events, provides tours, performs referral source in services, acts as a liaison between the referral source and the RRH, and seeks out new business development opportunities for RRH. Frequent travel to various agencies and facilities within the community.

Must be able to work some evenings and weekends.

• Education per RN Licensure requirements, BSN preferred
• Minimum of 2 years rehabilitation experience in a rehab practice setting preferred. Previous marketing experience preferred. Rehab settings may include inpatient rehab, long term facility, home health or general hospital settings.
