ASSESSMENT / REFERRAL COORDINATOR - ACUTE REHAB - PART TIME
Texoma Medical Center is a 414‑bed acute care facility located in Denison, Texas, and part of Universal Health Services, a Fortune 500 health‑care provider. The position involves coordinating patient assessment and referral within acute rehabilitation services.
ResponsibilitiesThe basic purpose of this role is to provide safe, competent assessment and referral services for patients in accordance with facility policies and standards.
Responsibilities include:
identifying referral sources, evaluating patient needs, developing appropriate plans, performing outreach and marketing activities, and maintaining long‑term relationships with referring agencies. The coordinator will also participate in community events, provide tours, and act as a liaison between the referral source and the rehabilitation center. Occasional evening and weekend work and frequent travel to agencies within the community are required.

- Education per RN licensure requirements, BSN preferred
- Minimum of 2 years rehabilitation experience preferred; prior marketing experience a plus. Settings may include inpatient rehab, long‑term facility, home health or general hospital.
- Valid, current RN state license. BLS course required; ACLS and CRRN preferred.
- Excellent interpersonal, communication and problem‑solving skills; ability to gather and exchange data internally and externally. Knowledge of CMS and JCAHO standards. Self‑motivated, personable, and strong verbal and written communication.
