Registered Nurse (RN) – Full‑Time – Per‑Visit – Home Health

Location: Denton, TX 762025
Sign‑on Bonus: $2,500

We are looking for an experienced Registered Nurse (RN) to provide skilled nursing care in the home health setting. The RN will see all visits assigned within the scope of practice of their license, including OASIS visits, skilled nursing visits, and supervised visits. The RN will also participate in the compensated on‑call program.

• Case management and supervised visits.
• Perform skilled and comprehensive assessment of the patient during the initial admission.
• Assess and chart observations of the patient’s condition at each visit (vital signs).
• Provide skilled nursing care as needed in accordance with the plan of care (administering medication, cleaning/dressing wounds, assessing the healing progress).
• Provide proper patient education for home health care procedures and strategies.
• Responsible for the overall clinical record maintenance to include visit note (must be completed within 24 hours), comprehensive assessments, including OASIS (must be completed within 48 hours), medication profile, orders/plan submitted in a timely manner, etc.
• Maintain confidentiality of patient PHI through HIPAA law.
• Keep the patient comfortable through palliative care.

• Current Texas State license as a Registered Nurse.
• 2+ years experience as a RN in Home Health.
• Current CPR certification.
• Reliable transportation.
• Texas Driver’s License and Auto Liability Insurance.
• Current TB Skin Test.
• EMR experience (Kinsser preferred).

Minimum productivity requirement: 32 productivity points weekly.
