Stability Focused Case Manager at Colorado Village Collaborative

Pay$25.00 per hour; benefits: 401k matching, 100% employer-paid health, dental, vision; paid vacation, paid holidays; $40 cell phone stipend
Schedule40 hours per week; flexibility for occasional evenings and weekends
Key Responsibilities- Individual Case Management
- Conduct program assessments and collaborate with clients to set goals.
- Assist with identification document recovery for benefits, employment, and housing.
- Provide strengths‑based, trauma‑informed case management using motivational interviewing and problem‑solving supports.
- Connect clients to health, behavioral health, harm reduction, substance use, vocational, and employment services across the All In Mile High system.
- Make referrals to outside agencies and internal colleagues, such as Peer Support or on‑site Mental Health Professionals.
- Help clients access insurance and primary, dental, and specialty care.
- Transport community members as necessary using personal vehicle.
- Benefits and Resource Navigation / Income Stability
- Assist clients in applying for and maintaining public benefits such as SNAP, Medicaid, and SSI/SSDI.
- Coordinate with benefits specialists and partner agencies for timely submission and follow‑through.
- Support job searches, resume development, interview preparation, and connections to hiring events.
- Facilitate access to vocational, educational, and workforce development opportunities.
- Educate clients on navigating systems to maintain long‑term stability and self‑sufficiency.
- Stay informed of current trends in community resources and best practices for resource access.
- Data Collection and Documentation
- Maintain accurate HMIS data, case notes, and community member documentation.
- Ensure proper documentation of all activities, communications, and progress.
- Complete all required forms and reports on time per program requirements.
- Professionalism and Teamwork
- Participate in case conferences, team meetings, training sessions, and supervisions.
- Collaborate with internal teams and community partners to support stability goals.
- Promote a collaborative culture that respects client self‑determination and decision‑making.
- Follow related policies and procedures of the Micro Community operations.
- Other Responsibilities
- Complete special duties and tasks as assigned by management.
- Support the dynamic and growing team by contributing to overall success.
- High School Diploma or equivalent; one year of experience or volunteering with unhoused individuals in a non‑profit setting.
- Preferred additional education or experience in social work, human services, or related fields.
- Experience in case management, benefits navigation, homelessness services, or related fields with understanding of Housing First, harm reduction, and trauma‑informed engagement.
- Ability to work within community locations, shelters, and office sites.
- Familiarity with HMIS or similar data systems a plus.
- Reliable transportation required.
- Cell phone required to access and download applications.
- Cultural competence and empathy for diverse backgrounds.
- Excellent verbal and written communication skills.
- Strong organizational and prioritization abilities.
- Creative problem‑solving and flexibility.
- Proficient data management and record‑keeping.
- Collaborative teamwork across stakeholders.
- Trauma‑informed approach to client interactions.
- Comfort with driving and transporting clients.
The position requires the ability to perform essential functions with or without reasonable accommodation. Tasks involve prolonged standing, walking, sitting, and may require reaching, bending, crouching, kneeling, and manual dexterity. Visual and auditory ability to perform tasks and ensure safety is essential.
