Supervisor, Prevention Navigation is responsible for developing and implementing strategies to engage and link individuals to Vivent Health Prevention Navigation services. The Prevention Navigation program provides a client‑centered approach to helping clients reduce barriers and build the skills needed to access services such as medical insurance, hepatitis C care, and addiction treatment services for those who are ready. This includes testing new approaches, collaborations, and other prevention strategies;

developing partnerships with community organizations; monitoring program success; and supervising the prevention navigation team. We offer a welcoming and judgement‑free environment utilizing harm‑reduction strategies.

  • Lead, recruit, train, supervise, coach, and evaluate staff and volunteers to implement programs in accordance with state requirements and guidelines, and within the department budget and operational plan parameters. When personnel problems occur, consult with the department director and human resources and take appropriate action.
  • Serve as a member of the Prevention leadership team, which includes providing input, engaging in discussion, maintaining consistent programming, and carrying out Vivent Health’s mission and market culture with team members.
  • Integrate referral strategies into prevention service delivery systems (e.g., HIV counselling and testing, Lifepoint syringe access programming, education, outreach programs).
  • Build relationships and work with internal and external partners to develop and implement new strategies and warm hand‑off referral systems.
  • Utilize electronic prevention records to generate program statistical reports and complete grant progress reports as required.
  • Develop program metrics and conduct ongoing assessments to ensure program success.
  • Provide outreach to all relevant communities with a focus on people who inject drugs (PWID).
  • Develop and monitor the annual programmatic budget and work plans.
