• Licensed Occupational Therapist
• Open to applicants of all experience levels

• Screen/evaluate patients with cognitive and/or ADL physical functioning disorders and develop appropriate plans of care following regulatory and clinical standards.
• Utilize standardized assessments and evidence‑based practice to support clinical interventions.
• Provide rehabilitative, skilled and medically necessary treatment interventions per regulatory and clinical requirements.
• Administer rehabilitation procedures including ADL retraining, therapeutic activities, exercises, modalities, and assistive devices.
• Assess patient progress and adjust treatments accordingly in the clinical record.
• Evaluate treatment outcomes for effectiveness; modify services and refer as appropriate.
• Provide consultation and counseling to patients, families, caregivers, and other service providers.
• Communicate with supervisor and health team about patient progress, barriers, and treatment plans.
• Provide comprehensive discharge summary of services per regulatory requirements.
