Base pay range

$23.00/hr - $27.50/hr

JOB SUMMARY

The focus of this position is to ensure the efficient delivery of quality case management services in English and Spanish to meet the needs of Tepeyac patients. If a language other than English or Spanish is preferred by the patient, Case Managers refer to the use of the Limited English Proficiency (LEP) Language line for assistance. CMs provide services focused on patients with chronic health condition (i.e. diabetes, HIV), a behavioral health challenge, substance use concern, pediatric/OB patients, etc.

The most common case management tasks are connecting patients with valuable community resources, assistance with medication adherence and monitoring, transportation coordination, food assistance, referral to legal services, etc.

PAY & BENEFITS

Tepeyac offers a full compensation package to all full-time employees. We are searching for two Case Managers to join our Tepeyac Team. The decision for which level the finalists will be offered is dependent upon the candidates’ education and experience.

The pay will be commensurate based on the years of experience.

  • Case Manager I: $23-$24.50/hour
  • Case Manager II: $24.50-$26/hour
  • Case Manager III: $26-$27.50/hour
MINIMUM QUALIFICATIONS
  • Case Manager I: 1 year of experience working as a case manager. Experience in medicine, psychology, social services and particularly working with marginalized populations highly desired. Patient Navigation and/or Motivational Interviewing Training preferred. Experience working with and/or knowledge of the Ryan White program is preferred.
  • Case Manager II: 2 years of experience working as a case manager. Experience in medicine, psychology, social services and particularly working with marginalized populations highly desired. Patient Navigation and/or Motivational Interviewing Training preferred. Experience working with and/or knowledge of the Ryan White program is preferred.
  • Case Manager III: 3 or more years of experience working as a case manager. Experience in medicine, psychology, social services and particularly working with marginalized populations highly desired. Patient Navigation and/or Motivational Interviewing Training preferred.
    Experience working with the Ryan White program is required.
  • All Case Managers Are Required
  • Education:

    BA/BS required in social work, human services, or related field. MSW/LCSW also appropriate.
  • Language:
    Oral and written fluency in English and oral fluency in Spanish required.
  • Supervision:
    No direct supervision of staff. May support learning and consultation for staff navigating patient needs. May mentor, train, and report on CM students.
  • Ideal Candidate:
    Strong initiative and passion to provide healthcare to low income, diverse populations. Creative thinking and is open to working in common work spaces.
ESSENTIAL DUTIES & RESPONSIBILITIES
  • Participate as key member of an integrated care model of health care delivery.
  • Coordinate client care with external service providers and ensure patient needs have been met to the best of your ability. Monitor client stability and self-sufficiency to provide other services as identified.
  • Develop therapeutic relationships with clients and work from a trauma-informed, strength-based, and client-centered perspective. Focus on what people can do, not on what they cannot do.
  • May participate and facilitate groups that support patients’ commitment to their health.
  • Provide referrals and link clients to agencies, community and government services and monitor referral completion.
