We’re seeking a Clinical Cardiology Pharmacist to deliver high-impact, outpatient-focused care through medication management, therapy optimization, and patient education. In this role, you’ll collaborate closely with a multidisciplinary care team to improve outcomes, strengthen transitions of care, and ensure safe, evidence-based medication use.
What you’ll do- Collaborate with multidisciplinary teams to optimize drug therapy and therapeutic outcomes
- Provide direct patient care: medication assessments, management, reconciliation, and patient education
- Clinically evaluate patients daily within your assigned service area
- Document pharmacy recommendations, plans, interventions, and med rec in the medical record
- Review medication regimens for indication, dosing, allergies, interactions, labs, duplication, and documentation gaps
- Provide timely drug information to physicians, nurses, and other healthcare professionals
- Respond to urgent/emergent events (e.g., level 1 trauma, code blue, massive transfusion)
- Support antibiotic stewardship, pharmacokinetic dosing, renal/hepatic adjustments, and monitoring
- Develop, implement, and monitor clinical protocols
- Process orders/prescriptions and support medication/supply needs as ordered by authorized providers
- Facilitate discharge planning and smoother transitions of care from a pharmacy perspective
- Promote cost-effective drug utilization and monitor polypharmacy/high-dose therapy to prevent adverse effects
- Communicate professionally with patients, families, and clinical/support staff
- Education & training (one of the following):
- ASHP-accredited PGY1 + ASHP-accredited PGY2 in Emergency Medicine or Critical Care
- ASHP-accredited PGY1 + 3 years practical clinical experience in Emergency Medicine with BPS board certification
- AHA Basic Life Support (BLS)
- AHA Advanced Cardiovascular Life Support (ACLS)
- Pharmacy degree from an ACPE-accredited program
- Minimum 1 year of pharmacy experience within the last 3 years
- High reliability and dependability
- Valid driver’s license
- Ability to perform repetitive motion (standing/walking/sitting/bending) and lift up to 20 lbs as needed
- Must meet ASHP-Qualified Residency Preceptor standards within one year of employment
This is an on-site role in a medical facility. Driving may be required.
