Position Summary:

Clinical Case Managers provide comprehensive psychological and behavioral case management services within a continuum of high to low intensity services designed to improve or maintain each individual person we serve’s abilities to function in the community. Assesses, monitors, and follows up with individual’s ability to meet basic needs in moving toward recovery. Responsible for all clinical documentation of each interaction with people we serve.

As a Clinical Case Manager, you will be providing transportation to the people we serve in your vehicle (additional cost of insurance and mileage reimbursed). This role will serve as a Clinical Case Manager to the 2 nd Judicial District’s Frequent Service User Enhancements (FUSE) Program as well as other criminal justice specific referral sources. The case manager will take both HITT (high intensity treatment team) and CTT (community treatment team) persons we serve.

  • CMI -$30.12/hr - $62,650 annually (Bachelor's Degree)
  • CMII -$31.25/hr.

    - $65,000 annually (Master's Degree)
  • Language Differential - $1hr - $2.50/hr for bilingual proficiency in Spanish & ASL

The posted range represents the full hiring range. Offers will not exceed this range, the majority of candidates are placed near the midpoint based on experience and qualifications.

Job Duties & Functions:

  • People we serve meet with a multidisciplinary team as they work towards wellbeing. Within the team there are case managers, nurse practitioners and/or psychiatrists, nurses, and others as needed to support an individual’s needs.
  • Case managers are the main point of contact for the people on their caseloads. This role has an average caseload size of 23 (changing as agency needs change). This team specifically works with justice-involved adults with behavioral health needs.
  • At this level of care, case managers are primarily community based so that they can help people serve attend to their needs in their natural environments. Case managers transport the people on their caseloads to various locations/agencies as part of linkage to community resources.
  • Clinical Case Manager will conduct their own intakes (if eligible) in various settings, including detention facilities.
  • Job Duties and Functions
    • Assist caseload in meeting basic needs for food, clothing, shelter, personal safety, and general medical and dental care, and assist them with applications for income, medical, housing, or other benefits which they may need and to which they are entitled. Identify persons for whom establishment of a protective payee-ship is necessary and arrange this assistance. This position does carry a grant to help support caseload basic needs.
