Healthcare Nursing
Shared medical appointment lead clinician, Maternal and metabolic health
(Nurse practitioner or physician associate)
Why this role exists:Most of a pregnancy happens between appointments. That's when blood sugar and blood pressure drift, questions pile up, and new moms feel most alone. After delivery, too many women get one visit and then nothing, right when their risk of type 2 diabetes and heart disease is taking shape.
Renew Rx was built to fill that gap. We don't replace a woman's obstetrician or primary care clinician. We plug in alongside them as the program that turns their care plan into daily progress, with group visits, remote monitoring and a dedicated coach supporting her every day between appointments, through pregnancy, postpartum and beyond. Her own clinicians stay in charge, and we keep them informed.
We've served more than 1,000 women. In a study independently analyzed by University of Colorado Anschutz investigators, our patients with gestational diabetes had fewer C-sections and newborn intensive care stays than published benchmarks.
Our mission is to guide more than 100 million lives to better health, starting with mom and never stopping there.
What you'll do:You'll lead our virtual group visits, the heart of how we care for moms. A group visit brings about eight women together for 75 minutes. Each one is a billable medical visit for every patient in the room, and it gives each of them something a 15-minute appointment can't: time, peer support, and a clinician who knows her.
The 4th Trimester (Renewing Mom)A rolling, multi-week series of virtual group visits for new moms, which you co-lead with a RenewR Care Management Coach or a specialist such as a pelvic floor physical therapist. Together you cover blood pressure and glucose recovery, mental and physical healing, feeding, sleep, and getting back to her own health. Moms who have been through it call it life-changing.
Diabetes in pregnancyPatients join from diagnosis for group visits every two weeks through delivery. While a coach leads the group, you hold clinical breakouts: you review continuous al glucose monitor data in our dashboard, adjust each patient's plan, and manage medications, including insulin. Between visits, her Care Management Coach keeps her on track using our One Thing method, one small change at a time.
One-on-onevisits, as needed
Establish-care visits for new patients, and follow-up visits when a patient needs more than the group can offer.
Your responsibilities:- Lead the medical portion of each group visit and document each participant's individual care
- Conduct virtual establish-care and follow-up visits, focused on diabetes and high blood pressure in pregnancy and postpartum
- Set and sign the care plans, targets and standing orders that guide remote monitoring between visits (coaches and the app work within your orders and never set clinical targets)
- Review glucose, blood pressure and lifestyle data, and manage pregnancy-safe medications within protocol
- Work alongside each patient's prenatal provider and primary care clinician, who remain her treating clinicians, keeping them informed and coordinating when her plan changes (no obstetric management or procedures)
- Supervise the Care Management Coaches who co-lead your sessions, and stay available to them for questions, usually within 24 hours for non-urgent items
- Review and sign remote monitoring and care management documentation each month
- Help shape our protocols, safety thresholds and escalation pathways, and join quarterly quality and safety reviews
You'll focus on what you do best. Her prenatal and primary care stay with her own clinicians, so your time goes to the part of care that usually falls through the cracks: helping her follow through on her plan.
You'll spend your time on care, not paperwork. We built our own technology, including Al- assisted charting and custom templates, so documentation and data review take minutes.
You won't do it alone. Coaches handle the daily follow-up, and you're trained on the full program model, workflows and tools before your first session.
You'll build it with us. This isn't a finished program.
You'll work alongside our founder, a physician associate who leads group visits herself, and what you learn in real visits becomes how we care for every mom.
You'll grow as we grow. Hours, scope and leadership expand with the programs, from postpartum metabolic recovery to PCOS, weight management and menopause care, plus protocol…
