Adaptive Home Health, LLC is seeking a Physical Therapist (PT) to manage a field-based home health caseload in the Denver area. You will perform in-home assessments, deliver skilled PT visits, and develop plans of care focused on gait, transfers, balance, and functional mobility.
You will work with a supportive clinical team, document thoroughly in HCHB, and coordinate with RNs and physicians to optimize patient outcomes. Colorado PT licensure is required, with travel across assigned territory.
