RN - Admissions – Care Synergy
- Location: Denver, CO
- Status: Full-Time
- Schedule: Tuesday - Saturday, 10am - 7pm or Sunday - Thursday, 10am - 7pm
- Pay Range: $81,244 - $95,451
- Supplemental Pay: Paid Mileage/Shift Differentials/Stipends
- Milage & Expense Reimbursement: $0.625 per mile
The Denver Hospice is looking for an Admissions RN to assist with patient admissions, explaining services and HIS admission visits.
Key Responsibilities- Provide assessment, supportive care and education to prospective patients and families regarding hospice services.
- Determine hospice appropriateness and assists in admitting patients to the hospice program.
- Act as liaison with assigned hospitals, facilities and physicians.
- Assess and counsel patients and families during initial interviews to determine level of care and needs.
- Gather data, review charts, record information and provide education on hospice philosophy.
- Complete all admission paperwork and initiate the clinical record through the EMR with accurate knowledge of admission protocol.
- Assess and place orders for immediate medication needs and oversee placement of durable medical equipment, supplies and oxygen.
- Consult staff at hospitals, SNFs or ALFs to determine discharge orders and facilitate transition to home, SNF or other residence.
- Interface with referring physicians, nurses, ancillary providers and discharge planners to facilitate smooth transition into hospice for patients and families.
- Communicate pertinent patient/family information to interdisciplinary team and follow through to facilitate smooth transition.
- Perform other duties and responsibilities as assigned.
- Minimum Education:
Graduate of an accredited school of nursing; BSN preferred. - Minimum Experience:
1–2 years in hospice and/or palliative care, medical/surgical, public health, oncology, acute care or home health. - Required License:
Current Colorado RN licensure, valid Colorado State driver’s license and auto insurance. - CPR certification required.
- Ability to lift a minimum of 35 lbs.
