RN - Admissions – Care Synergy

• Location: Denver, CO
• Status: Full-Time
• Schedule: Tuesday - Saturday, 10am - 7pm or Sunday - Thursday, 10am - 7pm
• Pay Range: $81,244 - $95,451
• Supplemental Pay: Paid Mileage/Shift Differentials/Stipends
• Milage & Expense Reimbursement: $0.625 per mile

The Denver Hospice is looking for an Admissions RN to assist with patient admissions, explaining services and HIS admission visits.

• Provide assessment, supportive care and education to prospective patients and families regarding hospice services.
• Determine hospice appropriateness and assists in admitting patients to the hospice program.
• Act as liaison with assigned hospitals, facilities and physicians.
• Assess and counsel patients and families during initial interviews to determine level of care and needs.
• Gather data, review charts, record information and provide education on hospice philosophy.
• Complete all admission paperwork and initiate the clinical record through the EMR with accurate knowledge of admission protocol.
• Assess and place orders for immediate medication needs and oversee placement of durable medical equipment, supplies and oxygen.
• Consult staff at hospitals, SNFs or ALFs to determine discharge orders and facilitate transition to home, SNF or other residence.
• Interface with referring physicians, nurses, ancillary providers and discharge planners to facilitate smooth transition into hospice for patients and families.
• Communicate pertinent patient/family information to interdisciplinary team and follow through to facilitate smooth transition.
• Perform other duties and responsibilities as assigned.

• Minimum Education:
  
  Graduate of an accredited school of nursing; BSN preferred.
• Minimum Experience:
  
  1–2 years in hospice and/or palliative care, medical/surgical, public health, oncology, acute care or home health.
• Required License:
  Current Colorado RN licensure, valid Colorado State driver’s license and auto insurance.
• CPR certification required.

• Ability to lift a minimum of 35 lbs.
