The Colorado Coalition for the Homeless seeks an Licensed Practical Nurse (LPN) for our Recuperative Care Center to work collaboratively within an Integrated Health Care for the Homeless team to support participants in the Recuperative Care programs. The position is mainly responsible for performing both independent and delegated duties under the supervision of CCH Registered Nurses (RNs). This role is located at 2175 California St.,
Denver, Colorado 80204 and reports to the Recuperative Care Manager. The position is eligible for a weekend shift differential of 10% based on scheduled hours.

Essential Job Functions- Contributes to patients’ assessment by collecting data for analysis.
- Performs plan of care interventions, including specimen collection and wound care.
- Assists patients with activities of daily living, personal hygiene, elimination, nutrition, and ambulation when needed.
- Empowers patients by providing appropriate ongoing health education and information.
- Administers medications following written protocols or direct provider orders, understanding LPN scope limits.
- Collaborates with multidisciplinary health care teams.
- Provides nursing services as needed for other CCH programs.
- Preceptors new staff members and nursing students.
- Participates in team‑based or interdisciplinary quality improvement initiatives.
- Attends required staff meetings.
- Maintains EPM, EHR, and other documentation core competencies.
- Works collaboratively with all facility employees to ensure optimal operations and uninterrupted client services.
- Performs other duties as assigned.
- Active Licensed Practical Nurse license in Colorado.
- Passion for working with vulnerable populations with a focus on trauma‑informed care.
- Passion for working in a harm‑reduction environment with collaborative goal planning with patients.
- Experience in harm‑reduction environments preferred.
$24.77 – $29.73 per hour. Compensation range depends on total relevant years of experience.
