Position Overview

The Licensed Practical Nurse (LPN/LVN) provides and documents skilled nursing care under the supervision of a Registered Nurse, following the care plan and physician orders for each patient. The role requires maintaining confidentiality and professional boundaries at all times.

Essential Job Functions
  • Deliver and coordinate quality patient care in compliance with physician orders.
  • Continuously observe and assess patient condition, reporting changes to the supervisor or physician.
  • Document all activities, assessments, nursing actions and care coordination promptly.
  • Participate in and update the nursing care plan.
  • Take appropriate nursing actions based on assessment to achieve expected outcomes.
  • Recognize changes requiring intervention and implement preventive care.
  • Adhere to Aveanna policies, state and federal regulations, accrediting bodies and the Nurse Practice Act.
  • Use infection control measures per OSHA standards.
  • Educate patients and families on disease process, self‑care and prevention.
  • Maintain competencies through education and skill evaluations.
Qualifications
  • Active, unencumbered LPN license.
  • Current CPR certification (hands‑on).
  • TB skin test –2 step, current within last 12 months.
  • One year prior hands‑on nursing experience.
  • Reliable transportation.
  • Proficiency in clinical assessments, documentation and compliance with nursing policies.
  • Continuing education as required by state.
Requirements
  • Graduate of an accredited nursing school.
  • Current, unrestricted state license as a Licensed Nurse.
  • Current CPR certification.
  • Proficiency in clinical assessments, documentation, and nursing care policies.
Additional State‑Specific Requirements
  • South Carolina – One year of pediatrics experience.
  • California – One year of experience working under current nursing license.
  • Louisiana – One year of experience working as a licensed nurse.
  • Continuing education as required by state.
Preferences
  • Six months of recent experience as a Licensed Nurse in a clinical setting.
  • Home health experience.

Physical and Environmental Requirements
  • Speak, write, read, and understand English.
  • Travel as needed.
  • Lift 50 pounds.

  • Prolonged walking, standing, bending, kneeling, reaching, twisting.
  • Climb stairs and sit for extended periods.
  • Visual and hearing acuity; sufficient sense of smell and touch.
  • React physically and mentally to emergency situations.
  • Function in environments with potential allergens and infectious diseases.
Other Duties

  • Compliance with Medicare and Medicaid funds, including required vaccinations and testing.
