Home Health Registered Nurse (RN)

Base pay: $77,600.00–$ per year.

Per‑visit pay: $49.00–$69.00 per visit/unit.

• Provide admission, case management, and follow‑up skilled nursing visits for home‑health patients.
• Administer ongoing care and follow‑up as directed by the Clinical Manager.
• Confer with physicians to develop initial treatment plans based on orders and patient assessment; revise plans as needed.
• Coordinate care with allied health professionals (physical therapists, occupational therapists, home‑health aides, etc.).
• Report patient care, condition, and progress to the physician and Clinical Manager continuously.
• Implement patient care plans in partnership with patients and families to achieve optimal outcomes.
• Oversee and supervise the work of Home Health Aides, Certified Home Health Aides, and Personal Care Workers, ensuring documentation is completed correctly.
• Discharge patients after consultation with the physician and Clinical Manager, completing required clinical documentation.
• Prepare medical documentation for all patients, including case conferences, medication changes, re‑certifications, progress updates, and care‑plan modifications.

• Diploma, Associate’s, or Bachelor’s Degree in Nursing.
• Minimum of one year of nursing experience (strong med‑surg, ICU, ER, or acute experience preferred).
• Home‑health experience a plus.
• Current and unrestricted Registered Nurse licensure.
• Current CPR certification.
• Strong organizational and communication skills.
• Valid driver’s license, auto insurance, and reliable transportation.

40 hours per week.
