RN Case Manager - Home Health

AMI-Wellness is currently hiring for RN s to join our team in Denver, CO! The RN team members are responsible for coordinating care and support services, as well as providing hands on skilled nursing interventions that allow patients to remain in their home.

Compensation:

Starts at $100,000/ year + additional performance bonus plan of $10,000

Job Responsibilities:

Enables patients to stay in their homes by providing health and personal services, supporting patients and their families. You will succeed because you have Independence, Flexibility, Judgment, and good communication skills. Good organization and time management is an essential part of the job. You will conduct assessments, coordinate with interdisciplinary team members, and supervise para-professional staff. The RN case manager is responsible for both long and short term care planning and appropriate interventions.

Qualifications:

• Current Colorado RN License
• Current Drivers License
• Reliable transportation plan
• Compliance with COVID and Seasonal Influenza Vaccination requirements.
