Sage Group Health Care Staffing is seeking Hospice RN CM for a 13-week travel / contract position in Denver, CO. Accepting both local and traveler candidates.

Pay Rate:
Range $2040 to $2120 Weekly Gross pay (Non-taxable travel/housing and M&I stipend available for qualified/eligible travelers)
Mileage Reimbursement: $0.49/mile for distance covered between patient home appointments
Unit: Home Health Hospice
Shift: 8 Hours;
Days, 8a – 5p
Schedule: 40 hours / week guaranteed, 5 shifts per week, Mon-Fri, No weekend or on-call
Position Requirements :- Registered Nurse: 1 year
- Required - Recent/Current Home Health Nursing experience: 1 year within the last 3 years – Required
- Recent/Current Hospice Nursing experience: 1 year – Required
- Home Care Home Base(HCHB) charting
- Required - OASIS experience
- Highly preferred - Must have valid
Driver’s License and Personal Auto Insurance - Must be Covid Vaccinated
- ASN or an equivalent– Required
- BSN or an equivalent– Highly preferred
- RN License for Stateof CO or Compact
- Required - BLS Certification from AHA or American Red Cross
- Required
- Provide and coordinate care for assigned patients in comfort of their homes.
- Develop and implement nursing care plans including Start of Care and admissions as assigned.
- Support patient recovery and ability to stay at home - enabling them to spend quality timewith their loved ones and doing the activities they enjoy.
- Empower patients and families to reach healthcare goals by educating them on disease management.
- Provide Hospice nursing care and services within the standards and scope of nursing practice asdefined within the state Nurse Practice Act regulations, policies, procedures,and protocols, as well as facility and corporate policies and procedures.
- Ensure physicians’orders are legible if taken verbally and transcribed appropriately perpolicy and procedure.
- Ensure patients’status is assessed on an ongoing basis and pertinent information is gathered and documented.
- Notify physicians of significant changes in status.
- Document and complete charting for all patient visits within 24 hours.
- Provide bereavement support to patient’s families as needed.
