Position: Case Manager - Loretto Heights - 10655

The mission of the Colorado Coalition for the Homeless is to work collaboratively toward the prevention of homelessness and the creation of lasting solutions for homeless and at‑risk families, children, and individuals throughout Colorado. The Coalition advocates for and provides a continuum of housing and a variety of services to improve the health, well‑being, and stability of those it serves.

Provides comprehensive case management services to families participating in Family Support Services (FSS) housing programs. Works collaboratively with families and CCH teams towards the general goals of increasing family stability, improving family functioning and increasing the self‑sufficiency of the family. Provides case management and advocacy services and works collaboratively with FSS case management, clinical, and housing teams, as well as with property management.

Overall, the right candidate will contribute to a cross‑functional team that uses a collaborative approach and humor to problem‑solve crises, identify and implement proactive solutions to improve the quality of life for the families we serve.

Additional Requirement

  • Valid driver’s license required. This requirement may be waived, if necessary, based on overall candidate experience and current needs of the business.

Essential Job Functions
  • Provides strengths‑based case management to families participating in FSS Housing program with the goal to assist families to obtain and sustain housing, wellness and self‑sufficiency.
  • Assists with connection and referral to mainstream resources including health, mental health, and substance use treatment and accompanies clients on appointments for housing, benefits, medical and mental health needs, when appropriate.
  • Advocates for the unique needs of families assisting them to gain access to any and all benefits and resources for which they qualify in the community.
  • Communicates clinical assessments, interventions and ongoing client contact through timely, thorough record keeping, contact forms, and verbal communication.
  • Monitors client/family progress by regular, ongoing contact with families receiving services and contact with collaborative service providers both external and internal to CCH.
  • Transports clients using a CCH vehicle or an approved personal vehicle.
