Discharge Coordinator, 24 hours
Southern Champion Tray
MAIN FUNCTION: Serves as liaison between Case Management Department and Extended Care Facilities. Provides clerical support to the Case Management Department.
EDUCATION: High school education; additional courses in business and/or medical related area preferred.
EXPERIENCE: Minimum two-year office experience required; excellent interpersonal, telephone and organizational skills required.
