Registered Nurse (RN) / Licensed Practical Nurse (LPN) Responsibilities- Work with the intention of Improving Lives and Exceeding Expectations.

- Attend required meetings in person or via Zoom.
- Maintain current nursing license and CPR/Basic Life Saving certification.
- Monitor staff compliance to all State and Federal requirements including blood borne pathogens, hazardous materials, infection control, and fire safety.
- Record and monitor all progress of residents and communicate with physicians according to facility policies.
- Participate in the development, revision, and accuracy of resident care plans.
- Ensure that all medications and treatments within the Registered Nurse (RN) / LPN (Licensed Practical Nurse) scope of practice (i.e. IVs, etc.) are administered as ordered and in a timely fashion according to facility policies and schedules.
- Provide management of the nursing care to the residents.
- Perform other duties as required.
- Perform comprehensive patient assessments, including vital signs, physical examinations, and medical history documentation using electronic health record (EHR) systems such as Epic or Cerner.
- Administer medications, injections, IV infusions, and other treatments following established protocols and safety standards.
- Monitor patients’ conditions closely, recognizing signs of acute pain, infection, or deterioration requiring immediate intervention.
- Assist with diagnostic evaluations such as specimen collection, laboratory processing, and basic imaging procedures when necessary.
- Collaborate with interdisciplinary teams to develop and implement individualized care plans, discharge planning, and follow-up care strategies.
- Maintain accurate medical documentation in compliance with HIPAA regulations and facility policies.
- Provide specialized care for patients with complex needs including dementia, Alzheimer's disease, memory care, or those requiring ventilator management or tube feeding.
- Ensure infection control practices are rigorously followed to prevent healthcare-associated infections across all patient populations.
- Current Registered Nurse (RN) / Licensed Practical Nurse (LPN) license.
- Valid RN or LPN license in good standing with relevant state licensing boards.
- Prior experience in acute care settings such as hospitals (including ICU or emergency room), nursing homes, or long‑term care facilities is highly desirable.
- Proficiency with EMR/EHR systems like Epic, Cerner, or Athenahealth; familiarity with ICD coding (ICD‑9/10) is advantageous.
- Strong knowledge of physiology, anatomy, pharmacology, and medical terminology necessary for accurate patient assessment and medication administration.
- Experience working with diverse patient populations including seniors, individuals with developmental disabilities, or those requiring specialized dementia or memory care services.
- Ability to respond effectively in high‑pressure situations involving urgent care or trauma scenarios such as Level I or Level II trauma centers.
- Excellent communication skills coupled with a compassionate approach to patient care; ability to work independently during night shifts while maintaining team collaboration.
