ð Substantive / Fixed-Term Considered An exciting opportunity has arisen for a Consultant in COTE / Frailty (Geriatric Medicine) to join a progressive and supportive NHS team within a well-established district general hospital in the North West.

This post is ideal for a Consultant with a strong interest in Comprehensive Geriatric Assessment (CGA), acute frailty pathways, and multidisciplinary team (MDT) working, looking to make a meaningful impact on patient flow and quality of care.ð¹

The Role Consultant-led COTE / Frailty service

Delivery of Comprehensive Geriatric Assessment (CGA)
Management of complex older adults with frailty, multimorbidity, and acute presentations
Close collaboration with Emergency Department, Acute Medicine, Therapies, and Community teams
Participation in the General Internal Medicine on-call rota (details available on request)
Active involvement in governance, audit, teaching, and service development
ð¹ The Service Established frailty pathways and structured MDT model
Strong integration with acute and community services
Focus on admission avoidance, early supported discharge, and patient-centred care
Supportive leadership with opportunities to shape and expand frailty services
ð¹

Essential Criteria GMC Registration with Licence to Practise
CCT in Geriatric Medicine / General Internal Medicine (or equivalent)

Experience with in Acute Frailty / COTE services
Commitment to MDT working and quality improvement
