Physician Associate - Home Visit Service

Job summary

Derbyshire Dales Primary Care Network are seeking a motivated and compassionate Physician
Associate (PA) to join our dedicated Home Visiting Service. Working
as part of a multidisciplinary primary care team, you will provide high-quality
clinical assessment, diagnosis, management, and follow-up care to patients
within their own homes.

This is an exciting opportunity for a skilled
clinician who enjoys autonomous working while remaining supported by
experienced GPs and other healthcare professionals. You will play a pivotal
role in improving access to care, reducing unnecessary hospital admissions, and
supporting some of the most vulnerable members of our community.

Main duties of the jobKey Responsibilities Conduct comprehensive clinical assessments in patients' homes.

Obtain and document detailed medical histories.

Develop and implement management plans within your scope of practice.

Request and interpret appropriate investigations.

Identify deteriorating patients and escalate concerns appropriately.

Prescribe through delegated pathways where locally agreed and in accordance with current regulations.

Work collaboratively with GPs, nurses, paramedics, pharmacists, and community teams.

Maintain accurate and timely clinical records.

Participate in clinical governance, audits, and quality improvement initiatives.

Promote patient education, self-management, and preventative healthcare.

The main functions of the role have been outlined above .Applicants will be expected to work in a rota that meets the operational needs of the DDPCN HVS and the eleven stakeholder practices.

The shifts will be Monday to Friday and more than likely the working day will commence around 0930 /1000 hours with a finish time of 1800 or 1830 depending on the rota setting within the wider team .The wider team includes GP's , ECP's, ANP and Paramedics .

Qualifications

EssentialMSc or Postgraduate Diploma in Physician Associate Studies or equivalent recognised qualification.

Successful completion of the Physician Associate National Examination.

Registration with the General Medical Council as a Physician Associate. Experience working in primary care, urgent care, community care, or a similar clinical setting.

Experience undertaking patient assessments and formulating management plans.

Experience working within a multidisciplinary healthcare team.

Knowledge & Skills Excellent clinical assessment and diagnostic skills.

Ability to recognise and manage acute and chronic presentations within scope of practice.

Strong communication and interpersonal skills.

Ability to work independently whilst recognising limitations and seeking support appropriately.
