Community Care Live is recruiting an Adult Social Worker for a Community Hospital Team to support discharge planning for adults in hospital settings. You will assess needs and coordinate appropriate support to promote independence and wellbeing, ensuring safe, person-centred discharge.
Applicants must have a Social Work degree and at least two years of post-qualification experience. Hospital/CHC experience is preferred.
