Support Worker, Direct Support Professional​/ Direct Care

What You Will Be Doing Providing practical and emotional support to individuals in line with their personal care plans Assisting with daily living activities including personal care, meal preparation, and medication support Supporting individuals to maintain their independence, dignity, and quality of life Encouraging participation in social, recreational, and community activities Building positive, trusting relationships with the people you support and their families Maintaining accurate and up-to-date records and documentation Working collaboratively with colleagues, healthcare professionals, and other agencies to deliver the best possible outcomes
