To provide a specialist speech and language therapy service as part of the Community Neurological Rehabilitation Team for adults within the tri-borough area of Hammersmith & Fulham, Kensington & Chelsea, and Westminster. The post holder will work in collaboration with the community teams within CLCH, other health care professionals, statutory and voluntary agencies to deliver excellent standards of patient care.

• The post holder will be based within one of the three MDT neurorehabilitation teams, and provide specialist assessment and treatment of clients with a wide range of acquired neurological conditions with swallowing and/or communication impairments. The post holder is also part of the Early Supported Discharge Team (ESD) for Stroke. This includes the opportunity to work as a service coordinator, triaging and managing ESD referrals from acute site partners.
• The post holder will be an integral member of the community neuro-rehabilitation team based in the specified borough, acting as a resource to other medical, nursing, therapy, social services and voluntary agency staff.
