As a Care Co-Ordinator, you will be responsible for assessing and coordinating the care needs of a designated caseload of service users and their carers.

You will plan and implement evidence-based care, work closely with patients and families, and provide professional advice and support to colleagues and partner organisations.

Working as a Band 6 clinician
, you will also support, supervise, and act as a professional resource for junior and less experienced staff.

• Assess and coordinate the care needs of service users and their carers.
• Develop and implement evidence-based care plans.
• Manage an allocated caseload effectively.
• Review and evaluate care requirements and outcomes.
• Work closely with multidisciplinary team members to provide coordinated care.
• Provide professional advice and support to other agencies and healthcare professionals.
• Supervise and support junior or less experienced colleagues.
• Promote high standards of clinical practice and patient-centred care.
• Contribute to the ongoing improvement of services.

You will work alongside a multidisciplinary team that may include Support Workers, Social Workers, Occupational Therapists, Psychologists, Employment Specialists, Family Therapists and Medical Professionals
.
