An exciting opportunity has arisen for an Assistant Practitioner to join our dedicated team supporting adults during inpatient stays at the Foxley Ward in Dereham. This is a rewarding role where you will work at the heart of a multi‑agency team to facilitate safe and timely discharges, ensuring individuals receive the right support at the right time.
AboutThe Role
Based within the hospital, you will collaborate closely with health and social care professionals to deliver high‑quality, person‑centred care. Our service operates under a Home First ethos, prioritising independence and recovery through strong partnership working.
As Part Of a Small, Dynamic Team, You Will- Undertake person‑centred assessments with patients and their families.
- Develop and implement care plans that promote independence and wellbeing.
- Work collaboratively with multi‑disciplinary teams to ensure smooth discharge planning.
To build meaningful relationships with the people we support, this role requires regular in‑person work in both the office and community.
We're looking to appoint either a Supported Assistant Practitioner (Scale

Our Supported Assistant Practitioners (Scale
F) join our 9‑month bespoke support programme (The Assistant Practitioner Development Programme), enabling you to progress to an Assistant Practitioner role (Scale G) subject to satisfactory programme completion.
