East of England Community Health and Care NHS Trust is seeking a Band 5 Community Nurse to join the Mid-Norfolk Community Nursing Team in Dereham. You will assess, plan and deliver care in patients' homes, clinics and care settings, focusing on holistic assessment and palliative/end-of-life and wound care needs.
The post requires a registered nurse with a driving licence and access to a vehicle. You will work across a 7-day rota, including weekends, supporting colleagues and students.
