The post holder will provide care co-ordination across the Northern Ireland Care and Research Network in accordance to the NICE Guidelines for Motor Neuron Disease (MND). The post holder will work within the newly established MND team based within the Western Health & Social Care Trust with close working relationships with the existing nurse-led care centre staff in the Belfast Health & Social Care team.
The post holder will provide a resource of specialist nursing knowledge and skills in the coordination of care and management of those affected by MND their families and professionals working with them. The post holder will work closely with the multidisciplinary teams and other agencies and will be involved in developing and implementing effective approaches to meet the needs of people affected by MND and their families.
The post holder will initiate and participate in audit, research and development of the Motor Neuron Disease service working across the appropriate teams. The post holder will be an advocate for patients/carers with MND acting as a specialist resource for healthcare professionals involved with these patients.
- Use highly specialist knowledge of MND, underpinned by theory, in order to care for this vulnerable patient group from diagnosis to end of life.
- Evidence of on-going personal and professional development within the last three years.
