We are currently inviting applications for a Community Staff Nurse to join our Community Nursing Team in North Bradford.
As a community nurse working within the planned care service, you will be providing high quality care for people in their own homes, ranging from long term conditions to complex and palliative care. You’ll be working on a rotational basis as part of a seven‑day service, with the support of agile technology to help you deliver timely, high quality care, with support from specialists across the Trust.
Shift patterns are between the hours of 8am and 9pm, 7 days per week.
There are lots of development opportunities available to you as a community staff nurse to enable you to progress your career and offer promotion to senior nurses or further study to become a qualified specialist practitioner or advanced practitioner, along with other modes of study to enhance your skills in tissue viability or palliative care as a few examples.
Applicants must be able to regularly travel extensively across the district to meet the needs and demands of the service.

To work as part of the district nursing team to ensure high quality health care is delivered to patients in their own home and/or clinic setting. To work in collaboration with the wider Primary Health Care team, whilst participating in the assessment of health needs and delivery of care to the practice population. The service is delivered over a 7-day rota and postholders are expected to work flexibly in line with agreed operating hours to cover a 24‑hour service.
