An exciting opportunity has arisen for an experienced and motivated Cardiac Rehabilitation Nurse Specialist to join our specialist Cardiac Rehabilitation Service on a 6‑month fixed term post. Secondment to this post will also be considered.
You will have a strong background and genuine passion in cardiology/cardiac rehabilitation and be knowledgeable in the management of patients following myocardial infarction (MI), coronary artery bypass graft (CABG), heart valve surgery, and heart failure.
The role involves delivery and development of cardiac rehabilitation programmes, supporting patients and their carers through education, secondary prevention, self‑management, and structured exercise programmes. You will oversee referral and caseload management, represent the service at multidisciplinary meetings, and work closely with key stakeholders to ensure safe, effective, and sustainable service delivery.
This is an excellent opportunity for an experienced cardiac rehabilitation specialist looking to further develop their knowledge and skills within a supportive cardiology team environment. You will be supported by an experienced Clinical Service Lead, a cardiology clinical specialist, and will work alongside a dedicated team committed to improving outcomes for cardiac patients across the community.
Main duties of the job- Provide clinical leadership within the multidisciplinary Cardiac Rehabilitation Team across Harrow and Brent, ensuring care is delivered in line with BACPR guidelines and evidence based practice.
- Lead/co‑lead the coordination and delivery of community based cardiac rehabilitation programmes and assessment clinics across a variety of settings.
- Support the professional development of staff, ensuring the team has the skills and knowledge required to meet the needs of patients and the service.
- Monitor service performance and contribute to achieving key performance indicators and BACPR accreditation standards.
- Represent the Cardiac Rehabilitation Service across the North West London cardiac network and relevant professional forums.
- Lead and contribute to audit, research, quality improvement, and service development initiatives, sharing outcomes through presentations and publications where appropriate.
- Support the development and implementation of patient pathways, clinical protocols, and service guidelines to promote high quality, consistent care.

- Registered Nurse or AHP Degree qualification or equivalent
- Current NMC/HCPC/GPhC registration (or equivalent)
- Independent Prescriber (or willingness to undertake)
- Membership of BACPR (British Association of Cardiac Prevention and Rehabilitation)
- Membership of a cardiac specialist interest group
- Post registration experience in cardiac rehabilitation
- Experience of working with cardiology patients
- Experience in leading a cardiac rehabilitation programme
- Experience of working within a community setting
- Demonstrates ability to develop new initiatives
- Demonstrates ability to manage change
- Experience in managing cardiac conditions including heart failure
- Evidence of experience in audit and research
- Ability to instigate appropriate situational decision‑making within the remits of the role based on sound risk assessment
- A working knowledge of clinical governance and evidence‑based practice
- Ability to demonstrate specialist clinical expertise
- Ability to work under pressure with competing priorities and to tight deadlines
- Sound working knowledge of guidelines in the field of cardiac rehabilitation and secondary prevention
- Knowledge awareness of recent NHS policies and agenda
- Ability to demonstrate an understanding of the holistic needs of the client group
This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
