Job Overview
This is an exciting time to join our Integrated Cluster Network Teams. The Integrated Teams work to support individuals and their families with complex physical, mental, and emotional challenges across both older and younger adults, ensuring voice, choice, independence and control. We are seeking a full‑time permanent Occupational Therapist to join our teams in Bridgend, working on a county‑wide basis to focus on hospital avoidance and independence strategies.
The post covers adults experiencing frailty, disability and cognitive challenges. The role supports both individuals and carers to maximise independence within managed care and to ensure commissioned care is proportional to each individual’s capabilities.
Key Responsibilities- Deliver evidence‑based, outcome‑focused assessments and interventions to support independence and hospital avoidance.
- Take personal responsibility for workload within the specialist OT area and promote outcome‑focused commissioning across the team.
- Collaborate with social workers, nurses, physiotherapists, dietitians, speech and language therapists, pharmacists, GPs, and community psychiatric nurses.
- Develop and implement innovative OT approaches to support residential and community settings.
- Build resilient community connections and groups in Bridgend to extend services.
- Provide OT expertise to other teams and promote best practice within the organization.
- Full‑time permanent OT position (37 hours per week).
- Strong commitment to strength‑based, outcome‑focused care and a continuity of care model.
- Ability to greet and communicate with clients in Welsh.
- Valid driving licence.
- Enhanced DBS check with Adults Barred list clearance.
- Commitment to the council’s Hybrid Working Policy.
