Center for Life Management (CLM) provides the area's most comprehensive array of outpatient mental health services for adults and elders. Our Integrated Care Program is a highly innovative approach to whole healthcare that encompasses the integration of treatment modalities for mental and physical health needs. The Integrated Care (IC) team utilizes a full wrap-around care model that is necessary to support individuals with addressing the intersection of their complex mental and chronic physical health conditions.
The Clinical Supervisor oversees clinical and administrative functions of designated clinical staff, including clinical interns. Additionally, they provide initial and ongoing diagnostic assessment, treatment planning, therapy, and crisis management to help client’s improve symptoms and behaviors that otherwise interfere with daily functioning and personal wellness goals. This is a full time,
40 hour per week position.
Functional Area
s and Description of Duties
- Provide new staff with orientation and ongoing clinical and administrative training opportunities to team.
- Provide formal, regularly scheduled supervision and supervisory support as needed.
- Provide ongoing feedback on performance and complete formal performance reviews.
- Work in conjunction with the Director of Integrated Care and the Director of Operations to ensure that service delivery is efficient and effective by developing methods to meet desired client outcomes, productivity, and budget variances.
- Provide set hours of billable service each week to meet 18.75% of direct service key performance indicator expectation (eligible for incentive pay upon exceeding productivity expectation).
- Ensure self and team members:
- Provide individual diagnostic assessment that incorporates physical health diagnoses and symptoms to determine client’s strengths, needs, resources, and objectives.
- Formulate treatment plans that are collaborative with primary and specialty care specifying presenting problems, objectives, interventions, and desired outcomes.
- Assess client needs and ensure that they receive high-quality care to improve their overall quality of life and co-occurring health conditions.
- Complete assessments for eligibility determination for State funded services.
- Provide individual and group psychotherapy (INDPSY, GRPPSY) in the office, community, and/or remote via telehealth to reduce and/or ameliorate symptoms and behaviors that interfere with functioning and goal achievement.
- Collaborate with families and/or external supports by providing family training and counseling services as needed to educate supports and promote how to best encourage the client’s resiliency and recovery toward treatment goals.
- Provide Crisis Intervention (CI) when clients experience acute exacerbation of symptoms to ensure their safety within the home and/or community.
- Work collaboratively with other treatment team members, primary care physicians, specialists, guardians, natural supports, and any other external providers to ensure a wrap-around approach is provided for continuity of client care.
- Adhere to and uphold the practices specific to the Integrated Care Model specific to support the authenticity of program implementation.
- Serve on appropriate committees, both internal and external, as determined by supervisor.
- Function professionally and in a manner that protects the integrity, confidentiality, and rights of all clients.
- Promote a positive work environment and culture that fosters cooperation, teamwork, professional growth and development to benefit CLM staff, clients, families, and community partners.
- Execute all required services and documentation in accordance with agency policies, federal standards, and state statutes.

- Maintain professional memberships, licenses, accreditation, certifications, etc.
