Center for Life Management (CLM) provides the area's most comprehensive array of outpatient mental health services for adults and elders. Our Integrated Care department seeks to reduce the premature mortality of individuals living with mental illness by providing chronic disease prevention and wrap-around chronic disease management services. This specialty care team helps clients build greater independence, reconnect with their communities, and access treatment in the least restrictive setting appropriate for them, while recognizing that clients living with complex or chronic medical conditions sometimes need us to bring care to them.
The Behavioral Health Visiting Nurse is responsible for providing community-based nursing services and outpatient community-based support to assist client’s with improving symptoms and behaviors that otherwise interfere with daily functioning, medical needs, and personal wellness goals. This is a full time, 35 hour per week position.
Functional Area
s and Description of Duties
- Provide nursing services in client’s homes or other community-based settings according to the objectives, interventions, and desired outcomes identified in the client’s treatment plan.
- Take vitals regularly and communicate results with internal and external medical providers.
- Provide education on chronic disease self-management.
- Provide Individual Therapeutic Behavioral Services (TBSI) to develop, apply, and reinforce coping strategies to reduce and/or ameliorate mental health symptoms and behaviors that interfere with functioning and goal achievement.
- Provide Family Training and Counseling (FTC) services as needed to educate natural supports and promote how to best encourage the client’s resiliency and recovery toward treatment goals.
- Provide Medication Training and Support (MTS) services as needed to support clients in maintaining their prescribed medication regimen while integrating behavioral tailoring strategies into daily routine to promote effective medication management.
- Provide crisis management and intervention (CI) when clients experience acute exacerbation of symptoms to ensure their safety within the home and/or community.
- Use clinical skills and available data to complete nursing assessments to guide individualized service provisions for Integrated Care clients.
- Coordinate medication refills and administer intramuscular injections as ordered by prescribing medical staff.
- Work collaboratively with other treatment team members, Healthy Together program, natural supports, and external providers, to ensure a wrap-around approach is provided for continuity of client care.
- Provide set hours of billable service each week. Eligible for incentive pay upon exceeding productivity expectation.
- Provide timely documentation on each service delivered.
- Serve on appropriate committees, both internally and externally, as determined by supervisor.
- Function professionally and in a manner that protects the integrity, confidentiality and rights of all clients.
- Execute all required services and documentation in accordance with agency policies and federal/state standards.
- Maintain professional memberships, licenses, accreditation, certifications, etc.
- Adhere to Center’s Code of Ethics.
- Represent CLM in a professional and appropriate manner in all settings.
- Perform all other duties as assigned by supervisor.
