Description
Responsible for the development and the provision of pharmaceutical care in collaboration with multidisciplinary healthcare teams. Works to promote the use of established medication use policies and guidelines, and assists with the development of new Pharmacy Department and Pharmacy and Therapeutics Committee policies. Provides education to patients, students, residents, pharmacists, and other health care professionals. Assumes a leadership role as an extension of their leader to promote clinical practice advancement in the profession of pharmacy while maintaining the highest level of professionalism and confidentiality.
EssentialKey Job Responsibilities
- Perform all duties of Clinical Pharmacists when staffing in designated patient care areas (40–80% of time).
- Establish effective relationships to provide high-quality care and provide leadership within the healthcare team for provision of clinical pharmacy services in a specific practice area, demonstrating professionalism and exhibiting the core values of Mercy One.
- Support the work of the Pharmacy and Therapeutic Committee, including planning, development, implementation, and maintenance of formulary at Mercy One, and participate in or lead local and national committees related to medication use within their specialty area.
- Conduct and/or assist pharmacy students/residents with Medication Use Evaluations and participate in departmental Quality Improvement efforts.
- Assist with monitoring and analysis of adverse drug events, including medication errors, for their specialty area and identify necessary process improvement and/or education of healthcare professionals.
- Fully understand the clinical and financial ramifications of services provided by pharmacy and understand the principle of formulary management and follow formularies procedures.
- Lead or assist with development, implementation of multidisciplinary medication-related protocols, guidelines, policies and advance pharmacy services for their specialty area while ensuring compliance with regulatory requirements.
- Assist with training and development of staff/clinical pharmacists as it relates to their specialty or patient care area, including development of competencies, training material, orientation, staff engagement and performance evaluation.
- Provide education to pharmacy, medical, nursing and other healthcare professionals, through lectures, literature reviews and other educational activities.
- Serve as preceptor or coordinate rotations for pharmacy students/residents, and support other preceptors on the pharmacist team.
- Perform other job functions as directed by Clinical Pharmacy Manager.

Minimum Qualifications RequiredEducation and Experience
- Pharmacy degree (BS or Pharm.
D.) from an ACPE approved college of pharmacy. - Successful completion of ASHP-accredited PGY-1 Pharmacy Practice Residency or equivalent experience preferred.
- Successful completion of PGY-2 Specialty residency or Fellowship or minimum three years of practice experience in areas of specialty practice.
- Licensed, or eligible for licensure, pharmacist in Iowa. Must be licensed within 3 months of hire.
- BLS certification within 3 months of hire.
- ACLS certification within 3 months of hire.
- BPS Board Certification in the area of specialty within 1 year of hire.
- Other certification if requested by direct supervisors/managers.
- Proof of completion of Mandatory Reporter abuse training for child and dependent adult abuse within three (3) months of hire.
- Demonstrates advanced knowledge in a specialty area and consults with healthcare team concerning the use, availability, dosage form, cost, side effects and possible incompatibilities of new and currently used drugs; takes the time to clearly explain information to patients.
- Effective communication with patients and other healthcare professionals.
- High level of analytical ability to research and evaluate drug information, interpret patient specific information, and resolve medication related problems.
- Proficient in use of computers, software for pharmacy and healthcare applications, specific pharmacy practice technology and equipment.
- Efficiently and adequately document clinical activities and interventions such as consults, patient education, and other patient specific information as defined by policy and procedure.
- Ability to utilize problem solving skills to promote a positive work environment and interpersonal relationships, while maintaining professionalism and confidentiality.
