Description

* Work hours 8:00am - 4:30pm.

Purpose:

The Healthy Start Care Coordinator supports the health and well-being of pregnant and parenting women, fathers/ partners, infants, and children (up to 18 months) through the Healthy Start program . This role primarily pr ovides one-on-one care coordination using a family-centered, strength-based, and trauma-informed approach.

Responsibilities:
  • Engage s in community outreach efforts, including canvassing, events, partnerships, targeted outreach, and other activities identified in th e H ealthy S tart outreach plan to recruit and retain program participants.
  • Provides individualized care coordination, including education, system navigation, referrals to community resources, and follow-up to ensure successful access to services .
  • Conduct s visits in participant homes, community settings, agency offices, or through virtual/phone formats.
  • Conducts screenings and develops, implements, and updates individualized care plans; monitors progress and adjusts strategies accordingly. Support s participants in navigating healthcare and social service system s , includ ing scheduling appointments and removing potential barriers.
  • Offer s education using evidence-based health and parenting curricula , as identified by the program .
  • Use s interpretation or translation services as needed to ensure accessible care.
  • Provides support to H ealthy S tart group-based health and parenting education sessions as needed, including creating a safe, welcoming environment that promotes learning and peer connection.
  • Coordinate s and/ or provide s group logistics , including enrollment, transportation (using an agency vehicle), child care, and interpretation.
  • Conducts individualized follow-up support as needed for group participants.
  • Represent s the program in the H ealthy S tart C onsortia and community collaborations as assigned.
  • Maintains timely , accurate , and confidential documentation in compliance with program standards. Participate s in team meetings, supervisions, trainings, and agency meetings as assigned.
  • Manage s time, schedule, and workload responsibilities effectively.
Qualifications:
  • High School Diploma/GED with 2 years of experience with young children, parents and/or families or an associate degree is required ;
    Bachelor’s degree is preferred
  • Ability to work respectfully with all population s
  • Ability to effectively engage with clients and community partners about program services
  • Good working knowledge of community resources and accessing these resources preferred
  • Complies with accepted professional standards and practice
  • Ability to work effectively with a multi-disciplinary team
  • Ability to work with the management information system and complete data entry
  • Mandatory Reporter of Child and Dependent Adult abuse training certification (within 30 days of hire)
  • Must have valid Iowa driver's license, auto liability insurance and reliable transportation if driving is required
  • Successful completion of pre-employment physical, drug screen, 2-step TB test and background check

  • Physical requirements:

    Light work, sitting most of the time, exerting up to 20 pounds of force frequently , and/or a small amount of force constantly to move objects .
