Mercy One Home Care – Des Moines is seeking a compassionate and dedicated Registered Nurse (RN) to join our home health team. In this role, you’ll provide high-quality, patient-centered care in the comfort of patients’ homes, helping them maintain their health, independence, and dignity.

Key Responsibilities
Conduct comprehensive assessments of patients’ health status and needs
Develop and implement individualized care plans in collaboration with interdisciplinary teams
Administer medications and treatments, monitor patient responses
Educate patients and families on disease management and health maintenance
Monitor vital signs and manage chronic conditions
Accurately document patient care activities
Coordinate care with physicians, therapists, and other healthcare professionals
Provide emotional support and counseling to patients and families
Ensure compliance with healthcare regulations and standards
Participate in quality improvement initiatives and ongoing education
Qualifications
Current RN licensure in the state of Iowa
BSN preferred; ADN accepted
Previous home health or community nursing experience preferred
Strong clinical assessment and critical thinking skills
Ability to work independently and manage time effectively
Valid driver’s license and reliable transportation
