Job Summary

The Registered Nurse (RN) is responsible for utilizing the nursing process (assessment, diagnosis, outcomes/planning, implementation and evaluation) to provide individualized nursing care to dialysis patients. Responsible for coordination of care, patient assessment, patient education and various other nursing interventions. Collaborates with physicians and interprofessional colleagues to plan, implement and evaluate dialysis care. Utilizes technology to enhance quality patient care, delegates to and supervises unlicensed personnel, and assumes overall nursing accountability for patient care.

Responsibilities
  • Provide direct nursing care to patients undergoing home dialysis.
  • Perform patient assessments and develop individualized care plans.
  • Educate patients and families on home dialysis procedures and equipment usage.
  • Coordinate with physicians and other health professionals to plan, implement and evaluate dialysis treatment.
  • Document patient history, symptoms, medications and care provided in compliance with applicable standards.
  • Supervise and delegate tasks to unlicensed personnel as appropriate.
  • Participate in on‑call rotation (Monday through Friday 8:00 am–4:30 pm schedule).
Qualifications
  • Bachelor’s Degree in Nursing preferred; RN license required in the State of practice.
  • Graduate from an accredited nursing program (AACN, ACEN, NLN CNEA).
  • Current unencumbered RN license with the State Board of Nursing; multistate licensure acceptable.
  • Maintain all required department specific licensure, competencies and certifications.
  • Demonstrated competency in using technology to enhance patient care.
  • Supplies full scope of nursing practice in accordance with Nurse Practice Act and Administrative Rules.
