- Correctly prepare all drugs and chemicals.
- Ensure compliance with standards of The Joint Commission, CMS and other regulatory bodies.
- Manage and administer the Adverse Drug Reaction Program.
- Develop and participate in new employee training and education programs.
- Participate in quality assurance programs related to drug utilization and effectiveness.
- Participate in patient drug therapy monitoring.
- Develop at least one clinically oriented program per year (e.g., Pharmacokinetic Dosing Service, Patient Counseling, Pain Management Services, or Nutritional Support Service).
- Promote an environment of continuous quality improvement.
- Teach and counsel patients and their families.
- Successfully complete all educational requirements for state licensure.
- Have a minimum of three (3) years of hospital experience within the last five years.
- Have BLS certification if required by state.
- Maintain a current license in the state of practice.
- Pharm D or BS Pharm plus advanced degree or completion of hospital pharmacy residency.
- At least one year in a managerial role.
- Board Certification and/or residency.
