Our Client has an immediate need for a Bachelor Level Case Manager in Boston, MA. SUMMARY OF CLIENT'S DESCRIPTION OF THIS OPPORTUNITY- Provide clients with clinical services, including case management, individual treatment or group work
- Complete all documentation including assessments, Relapse Prevention Plans, and Individual Treatment Plans
- Oversee the intake process
- Determine appropriate transition and discharge plans for all residents
- Develop individualized behavior management plans and support staff to understand the individual youth treatment concerns
- Facilitate case conferences and discharge planning for all residents
- Assist with family engagement, permanency planning and life-skills development for all residents.
- Provide on-call support
- Bachelors (Master's preferred) in social work or human services field
- At least 3 years of clinical experience
- Prior familiarity with Massachusetts child welfare systems and / or DCF
- Must have valid driver's license and vehicle
- Establish a relationship with one of the largest referral agency for behavioral-health
- Compensated weekly
