Bachelor Level Case Manager

• Provide clients with clinical services, including case management, individual treatment or group work
• Complete all documentation including assessments, Relapse Prevention Plans, and Individual Treatment Plans
• Oversee the intake process
• Determine appropriate transition and discharge plans for all residents
• Develop individualized behavior management plans and support staff to understand the individual youth treatment concerns
• Facilitate case conferences and discharge planning for all residents
• Assist with family engagement, permanency planning and life-skills development for all residents.
• Provide on-call support

• Bachelors (Master's preferred) in social work or human services field
• At least 3 years of clinical experience
• Prior familiarity with Massachusetts child welfare systems and / or DCF
• Must have valid driver's license and vehicle

• Establish a relationship with one of the largest referral agency for behavioral-health
• Compensated weekly
