Location: Northern Macomb County (cities include Armada, Ray, Macomb, Richmond, New Haven/Lenox, New Baltimore, Chesterfield, Columbus, Casco, Fair Haven, Ira).
Henry Ford Home Health Care provides comprehensive home health products and services, including chronic care management, joint replacement care, specialty infusion and transplant care, wound care, and palliative care. Occupational therapists conduct individual assessments, teach safe activity performance, provide adaptive equipment instruction, and manage home modifications. Initial OT visit occurs within 48 hours of hospital discharge, with therapy up to 9 weeks, 1–3 times per week as needed.
GeneralSummary
- Perform OT evaluation and treatment patient‑care duties.
- Communicate effectively with patients, coworkers, physicians, and other system employees regarding patient‑care needs and maintain accurate documentation.
- Supervise and delegate to assistants and technicians as appropriate.
- Complete necessary and accurate patient billing information.
- Bachelor’s Degree in Occupational Therapy.
- Master’s Degree preferred.
- Current State of Michigan Registration.
- National Board for Certification in Occupational Therapy (NBCOT).
- Patient‑care setting with potential exposure to communicable disease.
- Limited noise, temperature fluctuation, and dust possible.
